If you create a new button it does not show up on its bar until you move the bar's position using the preferences. (You can then move it back and the button will still be in view)

The button also disappears when you change certain settings in the package editor such as the auto size check box.

The button stuff in the settings editor has a lot of problems. I hope to get to the button problems after I finish with the multi-session stuff. But at this point, don't bother reporting button problems since there are many of them already on my bug list.
